Phase I - Comprehensive Assessment: Flat Rate of $1000.00. Life Transitions staff meets with client to determine what the client and/or family needs. It is during this phase that a comprehensive assessment is completed.
Phase II - Service Plan. Flat Rate of $500.00. Life Transitions offers a customized written plan with recommended service providers.
Phase III - Implementation of Plan. Hourly rate of $115.00. It is during this phase that Life Transitions staff assists as much or as little as the client wishes to implement the Phase II Service Plan.
Phase IV - Follow Up. No Charge. Six months to one year following the completion of the plan implementation, Anne K. Grasee will contact the client as a follow-up to determine quality of service provided.
Travel Charges: If travel is required outside the Denver metropolitan area, the client will be charged for air fare, lodging, and the travel time at one-half the normal hourly rate.
Billing: Invoices are mailed monthly. Payment is due upon receipt.
